If you have not previously attended our clinic for acupuntcure and have booked an Initial Appointment please print & fill out one of the below forms to speed up the intake process during your first visit.

If you do not have the ability to print it prior to coming in one will be provided to you to fill out when you come, but please come 10-15 minutes before your appointment start time!
